Nursing Facility Transition/Outreach Specialist

Position General Summary
This position provides direct service to individuals on Medicaid, who reside in Nursing Facilities (NF’s) and desire community-based living. The Nursing Facility Transition/ Outreach Specialist assesses the needs of customers, creates person-centered plans, assists customers in their move to the community, and provides follow-up services. Outreach on transition services is provided to persons residing in NF’s, to friends and family of individuals residing in NF’s, to NF staff, and groups in the community. The Nursing Facility Transition/Outreach Specialist also serves as an advocate at both the local and state level and provides information and referral services in their area of service as requested.
